Guidelines
- This form must be printed and filled in manually, in clear writing.
- A contact phone number must be given in case of a query or problem with the transaction.
- If paying by Laser no security number is needed, but the last three digits of the credit card must be given and can be manually written on payment form.
- A receipt will be sent out to the address given on this form and in the name of the DCU student.
