Request Name Change for Graduation 2026
Please complete this form if you wish to request a change to your correct registered name in advance of your graduation
Any student who has changed their name must submit one of the following documents as proof of this change:
- A valid passport (from your country of citizenship)
- A current driving license or learner permit (credit card format)
- Irish Certificate of Naturalisation
- Birth certificate
- National Identity Card for EU/EEA/Swiss Citizens
- Deed poll documentation.
- Gender recognition certificate
- Marriage certificate
