       IMPORTANT NOTICE: Please complete this form in full to register for the DCU Music Assessment Stage 2 for DC012 that will take place on 4th April DCU at the All Hallows Campus, Drumcondra .  

                    The form must be submitted by midnight on Monday 30th March 2020.

Please print and bring the form with you to the test.  You are also required to bring photo ID, preferably a current passport.
