Phonebook Photograph Consent Form
As part of the development work on the new DCU website, we have developed a new DCU Phone Book and People Search facility, designed to improve the search function when looking for contacts across campus. The design of this new phone book function allows for a photograph of each staff member along with their contact details, should they chose to have a picture up there. The purpose of having photographs available is to enable other staff members and guests to DCU to identify them when seeking out contacts in different faculties and departments.
These photos will ONLY be used on the DCU website and will not be used for any other purpose within DCU and the pictures will not be passed onto any third party.
Participation in the photo process is voluntary: you have the option to have your photo on the website or to not have your photo on the website. If you chose to have your photo on the website, you are entitled to have it removed at a later stage.
If you would like to participate and include a photo with your contact details, please complete the short form below. We will arrange for photography sessions to take place and you can have your new photo taken in a session that suits your schedule.
If you do not wish to participate and have your photo used in the phonebook, then no further action is required by you.
