You are requested not to apply on line. Print and submit completed forms to the Registry.

PLEASE NOTE:

HTML versions of the application forms are not presented as one complete web page, but as a series of linked web pages. You can navigate through a form using the NEXT and PREVIOUS links at the end of each page. Please ensure that you print and complete every page of the form.

In order to view these files you must have Adobe's Acrobat Reader installed. If you do not have this software click here.

To save a PDF document:

a) Open the PDF document by clicking on the link. Then go to 'File' in the menu bar and select 'Save'

b) Right click on the link and select 'Save link as' or 'Save Target as' . Note: you must have the ' .pdf ' file extension when saving the document to your local drive.
