The type of evidence required to register for DLSS services can be found HERE. This is in line with DARE and HEA funding requirements. 

Your GP may have evidence on file from your consultant which may be acceptable.

Data Protection Notice: By registering with the DLSS, you acknowledge that you will be also registering with Student Support & Development. By registering with both the DLSS and Student Support & Development your personal data, including sensitive categories of personal data (such as the nature of your disability or medical condition) will be obtained, processed and retained by the DLSS and Student Support & Development for purposes connected with the support services provided by the DLSS and the Student Support & Development. Your personal data will also be disclosed to the relevant academics and departments in DCU (e.g. Registry for Exam Supports) and to the Higher Education Authority (HEA) for funding purposes. Your personal data will be retained by DLSS until the end of the Operational Programme. The DLSS can be contacted on email:  disability.service@dcu.ie



Funding Application to FSD: I declare that all the information provided is true, complete and accurate and that assistance from other sources has not been received for the stated purpose/service which is the subject of the funding application to HEA (Fund for Students with Disabilities).
